Why:

When an execution by lethal injection in Oklahoma seemed to go badly wrong, state prison officials stopped the execution process. The prisoner died later of a heart attack. The prisoner's attorney said: "It was a horrible thing to witness. It was totally botched." Botch became the word used in many headlines and accounts of the incident.

Botch means "to do (something) badly" or "to ruin (something) because of carelessness or a lack of skill." Its original meaning was "to repair, mend or patch usually in a bungling clumsy inept way."
